At Naples Valley Chiropractic it is our desire to bring our patients the best health care possible with the least financial burden. We participate with most major insurers and have established the following policies to serve you best: Most insurance companies pay 80% of covered services rendered after the annual deductible has been met (if you have one). You are required to pay the other 20% or your established co-payment. We will complete our portion of all claim forms and submit to your insurance company. Your insurance company may not cover services they deem as "preventative, maintenance or elective care." If you elect to receive these services, you will be financially responsible for them. Injured workers are eligible for Workers' Compensation benefits and may receive chiropractic treatment. Workers' Compensation covers all examination, x-ray and treatment costs once treatment has been authorized. You must report your injury and file an incident report with your employer as soon as possible. If your employer's insurance carrier refuses to make payment, you may be financially responsible for your care or we may be able to submit to your group health insurance. |

No Fault insurance covers all examination, x-ray and treatment costs resulting from an auto accident as long as the insurance company has been notified in a reasonable amount of time (within 45 days after the date of accident in New York State). If for some reason the automobile insurance company does not pay, you may be financially responsible for your care or we may be able to submit to your group health insurance. Medicare pays a portion of treatment once the annual deductible has been met. Medicare does not cover physical examinations and adjunctive procedures such as physiotherapy. If you are due for an exam or elect to receive physiotherapy, you are financially responsible for these services. For those patients who have no insurance coverage, our policy is that treatment be paid for at the time of service. |
